Data Privacy (Republic Act No. 10173)

In compliance with the Data Privacy Act of 2012 (Republic Act No. 10173), our clinic is committed to protecting your personal information.

We collect your name, contact details, date of birth, address, government ID images, and photo solely to verify your identity during registration and to manage doctor appointments.
Government ID images are stored in a private, access-controlled system and are viewable only by authorized clinic staff for identity verification.
In line with data minimization under Republic Act No. 10173 (Data Privacy Act of 2012), government ID images are permanently deleted once your account is approved or rejected.
Your registration photo is retained in your patient record to help clinic staff identify you during visits and support physical verification at the clinic.
Your other personal data will be used for appointment booking, clinical records, billing, and clinic communications related to your care.
We do not sell or share your personal data with third parties except as required by law or with your consent.
You may request access to, correction of, or deletion of your personal data subject to legal and clinical record-keeping requirements under the Data Privacy Act of 2012.
